In the clip, Dr. Lee in the compacted contents of the blackheads to loosen them from their position. Then it presses on the mass with a comedone extractor and pulls it free. You can actually see the enlarged pore (hence the name) that has been left behind after all of the content it has pulled and scooped free.
“Oh, icky!” says the patient in response to seeing the crowd for herself.
Dr. Lee explains to the patient that the small plug of dead skin and oil is very superficial and the crater is only about a millimeter deep.
